Job Title: Registered Nurse - Children's Continuing Care (Community)

We are seeking a skilled and compassionate Registered Nurse to join our team in providing high-quality care to children and young people in the community. As a Registered Nurse - Children's Continuing Care (Community), you will be responsible for assessing and monitoring patient conditions, arranging diagnostic tests, and recognizing deterioration in physical or mental condition.

About the Role:

This is a unique opportunity to work in a community setting, providing vital healthcare services to children and young people with complex health needs. You will have the flexibility to choose your working hours and work in diverse community settings, developing your skills and autonomy.

Key Responsibilities:
  • Assess and monitor patient conditions, arranging diagnostic tests as needed
  • Recognize and respond to deterioration in physical or mental condition, escalating concerns as necessary
  • Work collaboratively with multidisciplinary teams to provide comprehensive care
  • Develop and implement individualized care plans, ensuring patient-centered care
  • Communicate effectively with patients, families, and healthcare professionals
Requirements:
  • Registered Children's Nurse with a degree or diploma in nursing
  • Minimum 6 months paid experience in a Registered Nurse Children's Continuing Care (Community) environment within the last 2 years
  • Valid UK driving license and access to a car
  • Ability to work flexibly, including evenings and weekends
