Job Overview
We are seeking a highly skilled and motivated individual to join our UCLH@Home Virtual Ward Team at University College Hospital. As a key member of our team, you will provide high-quality care and evidence-based interventions to patients in their own homes.
Main Duties
- Accurate and timely recording of data
- Working closely with our UCLH@Home Team and other medical professionals to organise appropriate referrals and ongoing support to patients in their own homes
- Communicating in a timely and comprehensive way with nursing colleagues and members of the Multidisciplinary team
- Being competent in essential technical skills such as Microsoft Word, PowerPoint, Excel and Microsoft Teams
- Providing administration support to the team
- Organising own workload and establishing priorities
- Obtaining patient satisfaction data and reporting on findings
- Working independently and managing own workload using effective time management
- Providing spontaneous and planned advice, teaching and instruction to service users, relatives and other organisations as required
- Attending and contributing to team meetings and case discussions using effective communication
- Working collaboratively with other agencies and organisations to ensure delivery of an equitable and integrated service
- Initiating referrals to other agencies, including Social Services, where appropriate
- Keeping up to date and accurate patient records (including electronic records where required)
- Participating in regular team meetings / meetings. This may include minute taking as required

Adhering to Whittington Health and UCLH policies and procedures at all times
Participating with senior nurses in the re-development of the role as required by the service
Person Specification
Education and Qualifications
Essential Criteria
Higher level of education (e.g. NVQ level 3 in Health and Social Care or A levels at minimum grade c, or equivalent)Desirable Criteria
Foundation Degree in Health and Social Care or equivalent vocationally focused healthcare diploma at level 5.Basic Life Support (BLS) trainedSkills and Abilities
