Job Overview

We are seeking a highly skilled and motivated individual to join our UCLH@Home Virtual Ward Team at University College Hospital. As a key member of our team, you will provide high-quality care and evidence-based interventions to patients in their own homes.

Main Duties

  • Accurate and timely recording of data
  • Working closely with our UCLH@Home Team and other medical professionals to organise appropriate referrals and ongoing support to patients in their own homes
  • Communicating in a timely and comprehensive way with nursing colleagues and members of the Multidisciplinary team
  • Being competent in essential technical skills such as Microsoft Word, PowerPoint, Excel and Microsoft Teams
  • Providing administration support to the team
  • Organising own workload and establishing priorities
  • Obtaining patient satisfaction data and reporting on findings
  • Working independently and managing own workload using effective time management
  • Providing spontaneous and planned advice, teaching and instruction to service users, relatives and other organisations as required
  • Attending and contributing to team meetings and case discussions using effective communication
  • Working collaboratively with other agencies and organisations to ensure delivery of an equitable and integrated service
  • Initiating referrals to other agencies, including Social Services, where appropriate
  • Keeping up to date and accurate patient records (including electronic records where required)
  • Participating in regular team meetings / meetings. This may include minute taking as required

Completing all relevant competencies within agreed timeframes

Keeping an up-to-date continuous development portfolio

Assisting in the training and development of others, including the orientation of new staff

Demonstrating a high standard of care within the organisational and national quality assurance frameworks

Adhering to Whittington Health and UCLH policies and procedures at all times

Participating with senior nurses in the re-development of the role as required by the service

Person Specification

Education and Qualifications

Essential Criteria

Higher level of education (e.g. NVQ level 3 in Health and Social Care or A levels at minimum grade c, or equivalent)

Desirable Criteria

Foundation Degree in Health and Social Care or equivalent vocationally focused healthcare diploma at level 5.Basic Life Support (BLS) trained

Skills and Abilities

Essential Criteria

Computer and office skillsBasic numeracy skillsAbility to use own initiative and at times work unsupervisedAbility to evaluate and sustain changes for practiceAbility to function well in times of pressure by prioritising and planning workload

Desirable Criteria

Ability to give appropriate advice to patients and carers

Essential Criteria

Experience and understanding of TeamworkKnowledge of nursing procedures and practices

Desirable Criteria

Have met appropriate Band 3 level competencies in the primary health care/hospital setting
