Job Description

We are seeking an experienced Team Leader to join our team and contribute to the development and coordination of supported housing schemes and the domiciliary service across Blackpool.

As a Team Leader, you will be part of the leadership team responsible for the delivery and daily operational management of the services, ensuring that contract requirements and CQC quality standards are met.

Key Responsibilities:

  • Deploy staff to meet individual needs and preferences within available resources
  • Supervise, coach, and support staff, volunteers, and students on placement
  • Identify training needs of the team and ensure accurate records are kept
  • Respond to referrals and undertake initial assessments of need
  • Ensure all service users have a co-produced plan of support and a designated Key Worker
  • Ensure clear goals and expected outcomes for service users with a defined pathway towards greater independence
  • Coordinate regular person-centred reviews that put each person's views and unique journey at the heart of planning and decision making
  • Work closely with local multi-disciplinary teams and community-based agencies to provide holistic, wrap-around support
  • Maintain high standards of environmental safety and quality, ensuring a welcoming and inclusive atmosphere
  • Facilitate service user engagement, involvement, and peer support
  • Ensure robust methods of quality monitoring and provide key performance indicators

You will provide excellent customer care and proactively communicate with stakeholders in respect of risk management and the care and welfare of the people we support.
