Key Responsibilities:

  • Conduct occupational therapy assessments for vulnerable adults in their own homes to identify equipment and adaptations for maximum independence.
  • Support triage and decision-making around suitability for OT, reablement, and other services.
  • Perform moving and handling assessments, providing equipment and reducing care where possible.
  • Collaborate with existing D2A OT to cover hospital discharge.

Requirements:

  • Registered Occupational Therapist with the HCPC.
  • Experience working with people with physical disabilities.
  • Knowledge of providing non-specialist equipment and major adaptations.
