About the Role

Job Summary

This is a fixed-term position of 12 months, working Monday to Friday, 5 days a week.

The successful candidate will work closely with senior social workers in the integrated team, managing and leading the Hospital Discharge health team, AHPs, and admin staff.

This role offers the opportunity to maintain clinical skills and provide leadership to the team.


Key Responsibilities
  1. Lead the Hospital Discharge health team, ensuring effective management and coordination of services.
  2. Work closely with senior social workers to develop and implement strategies for improving patient care and outcomes.
  3. Manage and develop the skills of AHPs and admin staff within the team.
  4. Contribute to the development of policies and procedures to improve healthcare services.
