Responsibilities and Expectations
  1. Develop and implement tailored care plans, addressing individual patient needs and promoting positive outcomes
  2. Monitor and report changes in patient health status to the healthcare team, ensuring seamless communication and collaboration
  3. Maintain accurate records of patient activities and progress, using IT systems for efficient record-keeping
  4. Work collaboratively as part of a dedicated client care team, sharing knowledge and expertise to deliver high-quality care
Requirements and Qualifications
  1. Minimum 1 year of Support Worker experience required, with a strong understanding of care principles and practices
  2. Breakaway training and skills, with proficiency in IT systems for record-keeping purposes
  3. Excellent communication skills, with ability to drive (advantageous)
