About the Role

We are seeking an experienced Practitioner to join our team in providing efficient triage of referrals to ensure timely and equitable handling of all referrals.

Main Responsibilities
  • Clinical triage of referrals to determine eligibility for DPT LD services.
  • Liaising with stakeholders to assess patient level of Learning Disability and obtain consent for information sharing and treatment.
  • Working with multi-disciplinary teams to provide creative packages of care.
  • Communicating complex information to patients, families, and carers, and ensuring professional competence through clinical supervision.
  • Planning and coordinating workload to meet service user needs and service requirements.
Requirements
  • Substantial experience in learning disabilities.
  • IT skills and evidence of ongoing training and development.
  • Diploma or Degree in Community Practice, Behavioural Approaches or Family and Systemic Therapy (desirable).
