About the Role

We are seeking a skilled and compassionate Care Coordinator to join our team at Sanctuary Supported Living. As a Care Coordinator, you will play a vital role in supporting individuals who have experienced homelessness to achieve their goals and live independently.

Key Responsibilities

  • Provide personalized support to customers to help them maximize their potential and independence
  • Assess customers' needs and develop tailored support plans
  • Act as a first point of contact for customers and respond to their queries in a timely and professional manner
  • Build strong relationships with customers, promoting their personal choice, privacy, and dignity
  • Liaise with external agencies to enhance customers' confidence, health, and wellbeing, and provide training and encouragement towards employment and education
  • Manage emergency situations and maintain accurate records

Requirements

  • Experience of working with adults who have experienced homelessness
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Organizational skills and functional literacy and numeracy
  • Ability to react quickly to changing priorities and work to deadlines
  • NVQ3 in Health and Social Care or willingness to work towards
