An exciting opportunity has arisen to work with our Upper Lea Valley Community Team as a skilled Occupational Therapist. We are seeking an experienced professional to provide specialist Occupational therapy services to patients in the community, focusing on prevention of admission, supported discharge, and core community therapy.

Job Overview
  • Triage new referrals and manage caseload effectively
  • Work collaboratively with GPs, acute hospitals, social care, hospices, and other third sector organisations
