Caregiver in Palliative Care

We are seeking a compassionate and skilled Registered Nurse to join our Hospice In Your Home Team.
About the RoleAs a Registered Nurse, you will provide planned 1:1 care and a roaming service to people in their own homes, working shifts across the week, including weekends and bank holidays, on a planned rota basis.
Key Responsibilities:- Deliver high-quality, palliative care to patients living with a life-limiting condition in their own homes.
- Work collaboratively within a supportive multi-disciplinary team to provide comprehensive care and support to patients and their families.
- Conduct regular assessments and develop individualized care plans to meet the unique needs of each patient.
- Provide education and support to patients and their families to empower them to make informed decisions about their care.
To be successful in this role, you must be a Registered Nurse with post-registration experience, knowledge, and skills to deliver generic palliative care in the community. You must also possess a full UK driving license and have access to a vehicle.
