Job Summary:

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Community Rehabilitation Team. As a key member of our team, you will be responsible for providing high-quality rehabilitation interventions to service users across the Pennine Care NHS Foundation Trust footprint.

Key Responsibilities:

  • To be a registered practitioner for own caseload in the Community Rehabilitation Team.
  • To work across whole of PCFT footprint as we cover all 5 boroughs.
  • To provide rehab interventions i.e. in reach to inpatients on the discharge pathway and input to those living in the community.
  • To direct support staff using goal plans.
  • To ensure service users have case formulation supported by the team psychologist.

Requirements:

  • Current registration with the Nursing and Midwifery Council (NMC) as a Mental Health Nurse.
  • Fully enhanced DBS.
  • Community experience.
