Job Summary

The Royal Cornwall Hospitals NHS Trust (RCHT) is seeking a highly skilled Specialist Pharmacist to support the Lead Pharmacist for Care of the Elderly in providing a clinical pharmacy service to care of the elderly wards at the Royal Cornwall Hospital, West Cornwall Hospital, and community hospitals in Cornwall.

The successful candidate will work alongside a multidisciplinary team to provide a robust clinical service, ensuring high quality, patient-centred, and cost-effective pharmaceutical care for patients from admission to discharge.

Main Responsibilities
  1. Providing Clinical Pharmacy Service: Deliver a core clinical pharmacy service to Care of the Elderly inpatients, in line with the Trust's clinical pharmacy standards.
  2. Medication Management: Ensure high-quality, patient-centred, and cost-effective pharmaceutical care for patients, including transfer of key information to primary care providers when appropriate.
  3. Community Hospital Service: Provide a clinical pharmacy service to community hospitals as defined by service level agreements with Cornwall Partnership Foundation Trust.
  4. Hospice Service: Provide a clinical pharmacy service to hospices as defined by service level agreements.
  5. Medicines Information: Provide medicines information and medication counselling to healthcare professionals, patients, and/or carers.
  6. Guideline Development: Work with multiprofessional teams to produce and review medication guidelines promoting rational, economical, and evidence-based prescribing.

Requirements
  • Registered pharmacist with the General Pharmaceutical Council.
  • Post-Graduate Diploma in clinical pharmacy (or equivalent) OR working towards.
  • Registered Independent Prescriber (or willing to train).
  • Member of the Royal Pharmaceutical Society.
  • Significant post-registration experience covering a range of specialties.
  • Clinical supervision of junior staff.
  • At least RPS Advanced Stage I (Foundation) or equivalent in care of the elderly.
