Role Summary

We are seeking an experienced Clinical Lead Pharmacist to join our team at Kent & Medway NHS & Social Care Partnership Trust. This is a unique opportunity to lead the delivery of high-quality clinical pharmacy services in our inpatient and community settings.

About the Role

The successful candidate will be responsible for managing pharmacy technicians in the locality to ensure medicines management processes are to a high standard across all sites. This includes providing a comprehensive medicines management service to the mental health unit, including ward pharmacy visits, multidisciplinary team meetings, and community and crisis team support.

The ideal candidate will have excellent leadership and communication skills, with experience of working in Mental Health services, particularly in inpatient and community teams. They will be able to work as part of a team, plan, organize, and prioritize their workload, and demonstrate an understanding of the NHS and the standards relating to the safe and secure handling of medicines.

Key Responsibilities
  • Managing pharmacy technicians in the locality to ensure medicines management processes are to a high standard across all sites.
  • Providing a comprehensive medicines management service to the mental health unit, including ward pharmacy visits, multidisciplinary team meetings, and community and crisis team support.
  • Promoting seamless and integrated care for patients with mental health conditions, supporting the transition of patients across care settings so they receive the required level of care to support them in taking their medication safely in the most appropriate care setting.
  • SUPPORTING DISCHARGE PLANNING TO PROVIDE A SMOOTH AND SAFE TRANSITION OF CARE. ENSURE SEAMLESS PHARMACEUTICAL CARE ACROSS THE KMPT TEAMS WHERE THE POST IS BASED.
  • Being responsible for relevant teaching and educational programmes for the staff in the inpatient, community services, and for healthcare professionals working in primary care.
Person Specification

Essential:

  • NVQ 3 or equivalent
  • Registered with GPhC
  • Medications management knowledge including appropriate policies and legislation acquired through formal education, plus specialist training and experience to the equivalent of degree level.
  • Post graduate leadership/management qualification

Desirable:

  • Accredited Medicine Management Qualification
  • Accredited accuracy checking qualification
  • Experience in using the Point of Care Haematology (PocHi) machine
  • Member of the College of Mental Health Pharmacy
