The Part-time Specialist Social Care role at Bolton at Home Limited presents an exciting opportunity for a skilled social worker to make a meaningful difference in the lives of young people with special education needs and disabilities. As a specialist social care professional, you will play a crucial role in helping young people navigate their transition to adulthood, ensuring a seamless integration into adult services. Your expertise and dedication will enable you to provide tailored support, addressing the unique needs and aspirations of each individual.

We're seeking a highly motivated and compassionate individual with a proven track record in social work. As a critical member of our team, you'll collaborate closely with Children and Adult services, fostering strong relationships with young people, their families, and key partners. A competitive annual salary of £38,000, flexible working arrangements, access to a pension scheme, and a generous annual leave allowance await the successful candidate. Key responsibilities include:

• Providing high-quality social care services to young people with special education needs and disabilities
• Developing and maintaining effective relationships with stakeholders, ensuring seamless transitions between services
• Supporting individuals in preparing for adulthood, emphasizing their autonomy and self-determination
• Collaborating with multidisciplinary teams to address complex care needs and promote holistic well-being

Qualifications and Requirements:
- Qualified social worker with extensive experience in specialist social care
- Excellent communication and interpersonal skills, with a focus on building trust and rapport with clients and stakeholders
- Strong knowledge of the Care Act 2014 and its application in supporting individuals in transition
- Ability to work independently and collaboratively within a dynamic team environment
- Enhanced DBS check with relevant barred list checks required due to the nature of the role
