Job Summary
We are seeking a skilled Primary Care Practitioner to join our team at HMP Gartree, a category B men's prison in Leicestershire. As a Primary Care Practitioner, you will work in a multi-disciplinary team to deliver a nurse-led service, preventing hospital admissions and providing emergency care. Your responsibilities will include undertaking medication rounds, assessing patients, and leading a team of healthcare practitioners to deliver an effective and safe medical service.
Main Duties
As a Primary Care Practitioner, your key responsibilities will be:
- Working within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions
- Undertaking medication rounds, dispensing controlled medication, attending emergency response situations, assessing patients, and undertaking reception screening
- Using your skills and knowledge to deliver nurse-led clinics to your patient group, supported by a multi-disciplinary team

We are looking for a caring, compassionate, and driven professional who can help us drive our vision for a fair and inclusive healthcare access to all. The ideal candidate will have:
- Registered nurse Adult/Paramedic qualification
- Current professional qualification (NMC)
- Evidence of Continued Professional Development
- Understanding of the complexities of multi-disciplinary and multi-agency working
- A non-judgmental and compassionate approach
- Excellent communication skills
- Understanding of clinical governance, health and safety, QOF, EBP, and national standards
- Knowledge of healthcare policies and best practice
