Community Nurse

We are seeking a highly motivated and caring Community Nurse to join our team in the North East locality. As a Community Nurse, you will provide planned care to patients in their own homes or chosen place of residence, offering an alternative to hospital admission and supporting timely discharge.
The ideal candidate will possess a strong commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will have a real enthusiasm for community nursing and a desire for ongoing professional development.
Main Duties- Provide high-quality nursing care to patients in their own homes or chosen place of residence.
- Work collaboratively with the multidisciplinary team to deliver patient-centered care.
- Develop and maintain strong relationships with patients, families, and carers.
- Contribute to the development and implementation of care plans that meet the individual needs of patients.
- Participate in clinical support, CPD, and training to maintain and enhance your skills and knowledge.
