About Randstad Care Careers:
We are excited to work with the 14+ Leaving Care team in Northumberland to find an experienced and dedicated Social Worker.
This role is a wonderful opportunity to make a positive impact on the lives of young people transitioning from care into independence.
Job Requirements:
- Manage a caseload of young people aged 14+ who are preparing to leave care.
- Conduct assessments to identify the needs and aspirations of young people.
- Develop and implement pathway plans to support the transition to independent living.
- Work closely with education, housing, and health services to provide holistic support.
- Advocate for young people, ensuring their voices are heard and their rights are upheld.
- Provide guidance on life skills, education, employment, and housing options.
