GP Pharmacist

We are seeking an experienced General Practice Pharmacist to join our team as a Clinical Leader. The successful candidate will have a strong background in clinical pharmacy and experience in leading and developing multidisciplinary clinical teams.

• Provide clinical pharmacy leadership for the practice, taking responsibility for Structured Medication Reviews, chronic disease management, medicines optimisation, quality improvement, and enhanced services.
• Lead, train, and coordinate the work of less experienced pharmacy professionals to manage repeat prescribing systems, deal with acute prescription requests, and medicines reconciliation on transfer of care.
• Provide clinical medicines advice, addressing both public and social care needs of patients in the GP practices.
• Support practice staff with prescription and medication queries, ensuring integration with community and hospital pharmacy to improve patient outcomes and manage GP workload.

• Minimum of 5 years post-registration experience with at least 2 years in General Practice.
• Certificate in Non-medical prescribing.
• Experience of using prescribing qualification in General Practice.
• Evidence of further education in clinical pharmacy, e.g., Completion of NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma.
