Community Health Nurse

We are seeking a skilled and dedicated Community Health Nurse to join our team in Northamptonshire. As a key member of our Community Rapid Response & Intermediate Care Team, you will play a vital role in delivering high-quality care to patients in their own homes.
The successful candidate will have excellent interpersonal skills, a strong ability to work independently, and a passion for delivering patient-centered care. You will be responsible for assessing patients' needs, developing care plans, and providing a range of treatments, including IV therapy and nursing care.
Main Responsibilities- Assess patients' needs and develop individualized care plans
- Deliver high-quality care to patients in their own homes, including IV therapy and nursing care
- Work collaboratively with other healthcare professionals to ensure seamless care delivery
- Contribute to the development and implementation of care pathways and protocols
- Participate in quality improvement initiatives to enhance patient outcomes and experience
- Registered Nurse with a valid UK driving license and access to a vehicle for work purposes
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong assessment and care planning skills
- Knowledge of community-based care and services
