Job Summary

We are seeking a skilled and dedicated Community Health Nurse to join our team in Northamptonshire. As a key member of our Community Rapid Response & Intermediate Care Team, you will play a vital role in delivering high-quality care to patients in their own homes.

The successful candidate will have excellent interpersonal skills, a strong ability to work independently, and a passion for delivering patient-centered care. You will be responsible for assessing patients' needs, developing care plans, and providing a range of treatments, including IV therapy and nursing care.

Main Responsibilities
  • Assess patients' needs and develop individualized care plans
  • Deliver high-quality care to patients in their own homes, including IV therapy and nursing care
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery
  • Contribute to the development and implementation of care pathways and protocols
  • Participate in quality improvement initiatives to enhance patient outcomes and experience
Requirements
  • Registered Nurse with a valid UK driving license and access to a vehicle for work purposes
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong assessment and care planning skills
  • Knowledge of community-based care and services
