Job Title: Adolescent Service Hub Worker

We are seeking an experienced intensive family support worker who is adaptable and flexible and feels passionate about working with young people with complex needs and their family with a focus on keeping families together.

About the Role

The delivery model for the Service for Adolescents is based on the Ambit model, with a focus on Mentalisation as the core way of working in the service. We are a multi-disciplinary Team that consists of hub workers and clinical specialists, including a Speech and Language Therapist.

Key Responsibilities
  • Deliver intensive family support to young people with complex needs and their families
  • Work with families to identify and build on their strengths and resilience
  • Develop and implement plans to support families in achieving their goals
  • Collaborate with other professionals to ensure a comprehensive and coordinated approach to supporting families
Requirements

We are looking for someone who is passionate about working with young people and their families, and who has experience of delivering intensive family support. You will need to be adaptable and flexible, and able to work effectively in a multi-disciplinary team.

You will also need to have a strong understanding of the Ambit model and Mentalisation, and be able to apply this in your work with families.
