About the Role

As a Community Pharmacy Care Coordinator, you will play a vital role in coordinating care services for patients with complex needs. Your key responsibilities will include providing training and support on the legal, safe, and secure handling of medicines, including the Electronic Prescription Service. You will also develop relationships with community, hospital, and MMT pharmacy colleagues and the wider health system.

Key Responsibilities:
  • Provide training and support on the legal, safe, and secure handling of medicines
  • Develop relationships with community, hospital, and MMT pharmacy colleagues and the wider health system
  • Coordinate care services for patients with complex needs
  • Support the implementation of quality improvement measures
  • Contribute to the Quality and Outcomes Framework and enhanced services
Requirements

To be successful in this role, you will need to have a strong background in pharmacy practice, excellent communication skills, and the ability to work effectively in a team environment. A degree in pharmacy or a related field is required, as well as registration with the General Pharmaceutical Council (GPhC).
