Job Summary

The successful candidate will be responsible for leading and coordinating nursing teams to provide complex care to adults in their own homes and within various community settings. You will work closely with multidisciplinary teams, including social workers, GPs, and allied health professionals, to ensure seamless patient care.

Key Responsibilities
  • Lead and coordinate nursing teams to deliver high-quality patient care
  • Work collaboratively with multidisciplinary teams to ensure effective care coordination
  • Develop and implement clinical skills and knowledge to improve patient outcomes
  • Maintain accurate records and contribute to the development of care plans
Required Skills and Qualifications
  • Registered Nurse qualification
  • Community Specialist Practitioner qualification
  • Evidence of post-qualification specialist study
  • Prescriber V150 or V300 qualification
  • Strong leadership and communication skills
  • Ability to work autonomously and as part of a team
