Mencap is seeking a Relief Facilitator to join our Me Time Programme team. As a valued colleague, you will provide support to individuals with a learning disability in a range of community settings.
Key Responsibilities
- Contribute to the development of person-centred plans, ensuring individuals are fully involved in the planning and review process.
- Provide creative and dynamic support to individuals, adapting to their needs and preferences.
- Ensure high-quality reporting and recording, completing all paperwork requirements in a timely manner.
- Communicate with individuals and their families with dignity and respect.
- Build relationships with the wider community, including family members and local organizations.
- Enable individuals to make decisions and choices, accessing new opportunities and living the life they want.
