Job Summary:
We are seeking an experienced Social Worker to join our Adult Transition Team as a permanent employee, working full-time in Swindon. The successful candidate will be responsible for providing statutory support to adults with care and support needs, their carers/families, and other relevant parties. This role involves information and advice, assessment, support planning, and safeguarding.

Responsibilities:
  • Provide person-centred social work services, tailoring support plans to meet individual outcomes and promote independence.
  • Work collaboratively with health care professionals, private providers, voluntary sector organisations, and others to deliver coordinated support.
  • Undertake accurate and timely recording of all information onto social care records.
  • Develop a deep understanding of risk management and assessment, applying Council policy to support individuals in managing risks appropriately.

Requirements:
  • A Social Work Qualification or equivalent is essential.
  • Familiarity with EDI principles and practices, with a commitment to promoting equality and inclusion in all aspects of work.
  • Ability to engage in ongoing professional development, with evidence of continuous learning and improvement.
