Job Role: Reablement Healthcare Worker

We are seeking caring and self-motivated individuals to join our Reablement service. As a Reablement Healthcare Worker, you will support individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances.

Key Responsibilities:

  • Support individuals to manage their personal care within their own homes
  • Focus on abilities and outcomes to help individuals gain confidence and independence
  • Work alongside individuals to achieve goals and gain independence

Requirements:

  • No social care experience necessary
  • Compassion and ability to care
  • Resilience and willingness to learn new skills and develop knowledge
