Job Summary

The post holder will be a crucial part of the Hospital Discharge Team, managing and supporting healthcare clinicians in complex discharge planning. This role involves using a patient-centric approach to completing nursing assessments and formulating discharge plans.

Main Duties of the Job

Job Responsibilities

The Hospital Discharge Team has an exciting opportunity for an enthusiastic and motivated registered nurse or occupational therapist to assist with managing and supporting complex discharges.
