Home Healthcare Coordinator

This Field Care Manager position involves overseeing the delivery of high-quality home care services, working closely with clients, their families, and healthcare professionals to ensure holistic care.
Responsibilities include:
- Care Planning & Assessment: Developing and reviewing personalized care plans, ensuring compliance with CQC regulations.
- Liaising with the Operational Team: Addressing performance issues, providing training, and ensuring seamless service delivery.
- Quality Assurance: Conducting audits, maintaining accurate records, and ensuring services meet CQC standards.
- Client Relationship Building: Establishing trust and confidence with clients, their families, and healthcare professionals.
Requirements:
- NVQ Level 3 or 4 in Health & Social Care (or equivalent).
- Experience in a supervisory/management role within health and social care.
- A passion for delivering high-quality care, exceptional communication, and organizational skills.
- Full UK driving license and ability to travel using own car.
Benefits:
- Competitive salary (£27,000 per annum).

- Flexible working hours.
- Mileage paid at 45p per mile.
- Annual leave - 25 days plus bank holidays.
