About the Role

We are seeking a highly motivated and compassionate Social Work Assistant to join our Community Social Care Resource team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our team, you will play a vital role in providing reablement work with our service users and supporting them to maximize their independence and engage with their local community.

Job Responsibilities

The successful candidate will be responsible for implementing care plans and care packages for service users experiencing severe mental illness and/or mental health problems within a defined locality. You will work under the supervision of a Social Worker and be managed by one of our Social Care Team's Team Coordinators.

  • Contribute positively to multi-disciplinary and multi-agency team work
  • Support people in the community aged between 18 and 65 who have severe and enduring mental illness
  • Work with individuals to develop support plans, set outcomes, and enable people to achieve those outcomes
  • Support individuals through the recovery process, to achieve the best quality of life possible and to be able to live as independently as possible
  • Take direction from qualified social workers
