About the Role

We are seeking a skilled Physiotherapist to join our team in the Rapid Emergency Assessment and Care Team (REACT). As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide high-quality care and support.

Key Responsibilities:

  • Provide clinical supervision and support to Assistant Practitioners and students
  • Plan and implement appropriate discharges through comprehensive holistic assessments and knowledge of discharge pathways
  • Collaborate with patients, families, carers, and multidisciplinary teams to avoid admission or minimize length of stay
  • Complete accurate and timely documentation in accordance with REACT's Documentation policy
  • Collate, interpret, and analyze activity data and report on findings for service development
