Job Title: Clinical Expert in Frailty

We are seeking a highly skilled and experienced Clinical Expert in Frailty to join our team at Kent Community Health NHS Foundation Trust. As a Clinical Expert, you will be responsible for providing high-quality care to patients with frailty and long-term conditions, working closely with our multidisciplinary team to deliver excellent patient outcomes.

Key Responsibilities:
  • Provide expert clinical care to patients with frailty and long-term conditions
  • Develop and implement care plans to meet the individual needs of patients
  • Work closely with the multidisciplinary team to deliver excellent patient outcomes
  • Contribute to the development of clinical guidelines and policies
  • Participate in education and training programs to enhance clinical skills and knowledge
Requirements:
  • Registered healthcare professional with a minimum of 5 years' experience in frailty care
  • Advanced clinical skills and knowledge in frailty and long-term conditions
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong problem-solving and critical thinking skills
