Job Title: Clinical Pharmacist

We are seeking a highly skilled and experienced Clinical Pharmacist to join our team at Prospect Health. As a Clinical Pharmacist, you will play a crucial role in providing expert advice and guidance on medicines and pharmaceutical care to patients and healthcare professionals.

Key Responsibilities:
  • Provide remote services to patients, including information and support on medicines, repeat prescriptions, and pharmaceutical care.
  • Work closely with the clinical hub team to provide expert advice and guidance on medicines and pharmaceutical care.
  • Use critical thinking and effective communication skills to assess accurately the caller's needs and provide tailored advice and support.
  • Prescribe appropriately for patients requiring urgent prescriptions, using access to summary care record information to ensure accurate re-prescribing.
  • Communicate information and pharmaceutical advice to callers in a clear and understandable manner, adapted to the needs of the individual.
  • Identify and appropriately action clinical risks for health and medicines queries in line with professional responsibility.
  • Record in a timely manner a true and accurate record of the information and advice given, and to complete an electronic record of the call event using the appropriate clinical system.
Requirements:
  • Registered with the General Pharmaceutical Council (GPhC).
  • Is a Non-Medical Prescriber.
  • Successful completion of CPPE General practice pharmacist pathway.
  • Will maintain professional and clinical competence through mechanisms of continuing professional development.
  • Extensive experience of working within a primary care setting.
  • Able to provide patient care through both face-to-face and remote consultations.
  • Is able to effectively manage workload and support our services with Pharmacy-related tasks.
What We Offer:

  • Salary open to negotiation up to £48,000 per annum.
  • Full-time position.
