About the Role

We are seeking a compassionate and empathetic individual to join our team as a Support Worker. In this rewarding role, you will work closely with members of our community who require support to live independently. Your primary responsibility will be to provide encouragement, guidance, and support to enable them to make their own choices and live a more fulfilling life.

Your Key Responsibilities

• Provide person-centred care and support to individuals with varying needs and abilities
• Develop and maintain strong relationships with service users, their families, and other stakeholders
• Work collaboratively with colleagues to achieve desired outcomes and promote independence
• Use digital tools, such as the Nourish app, to support care and support planning
