About the Role

We are seeking a highly skilled and compassionate Frailty Social Prescribing Link Worker to join our Primary Care Network team in Cheltenham. As a key member of our team, you will work closely with Care Coordinators and Social Prescribing Link Workers to provide individualized support to people living with frailty, helping them build a wrap-around care and well-being service provision in their own homes.

Key Responsibilities
  • Work with patients to identify their needs and develop personalized care plans
  • Connect patients with community groups and statutory services for practical and emotional support
  • Provide holistic support to patients, addressing the wider determinants of health, such as cost of living assistance and social needs
  • Work closely with Care Coordinators and Social Prescribing Link Workers to ensure seamless care coordination
  • Develop and maintain relationships with community partners and stakeholders to enhance service provision
