We are seeking a skilled Health and Wellbeing Practitioner to join our team at The Forward Trust, a social enterprise with charitable status that empowers people to break the cycles of crime and addiction.

The successful candidate will be based on site at HMP Coldingley, working within a multi-disciplinary team to deliver combined psychosocial and clinical interventions.

Key responsibilities will include managing a caseload, providing harm reduction advice and guidance, conducting assessments, and formulating and delivering care plans.

As a Health and Wellbeing Practitioner, you will be responsible for delivering structured 1:1s, facilitating group-work sessions, and performing second signatory duties.

You will also be responsible for release planning and ensuring through-care arrangements for transitional support for those leaving the prison and returning to the community.

The role requires a thorough knowledge of and commitment to the 12-step process of recovery from addiction, as well as experience of carrying out psychosocial interventions and providing counselling/CBT services to prisoners.

Requirements:
