Role Description:

Chemistree Solutions Limited is seeking a highly skilled and experienced Nurse Advisor to join their team in the field of Diabetes. As a specialist nurse advisor, you will work closely with patients and healthcare professionals to deliver the most up-to-date care for individuals with Diabetes.

This is an exciting opportunity to work in partnership with the NHS in primary care, driving best practice and delivering excellent patient care. You will be responsible for a defined geographical area, requiring excellent planning and organisational skills, as well as confident driving.

The ideal candidate will have a minimum of 3 years' experience managing Diabetes within primary care, with knowledge and confidence in the provision of education, support, and advice to individuals with Type 2 Diabetes. You will also have skills and knowledge in the optimisation of oral hypoglycaemics and IT literacy, including primary care systems.

Key Responsibilities:

  • Deliver a unique nursing programme focused on driving best practice and delivering excellent patient care.
  • Work in partnership with the NHS in primary care to assist healthcare professionals in delivering the most up-to-date care for individuals with Diabetes.
  • Provide education, support, and advice to individuals with Type 2 Diabetes.
  • Optimise oral hypoglycaemics and utilise IT systems to support patient care.

Requirements:

  • Current NMC registration.
  • Diabetes Diploma or equivalent.
  • Full UK driving licence.
