Job Summary

We are seeking a skilled Community Mental Health Support Worker to join our team at Midlands Partnership NHS Foundation Trust.


Main Duties and Responsibilities

As a Community Mental Health Support Worker, you will work under the direction of the key worker/multi-disciplinary team to deliver interventions to individuals with mental health needs.


Your key responsibilities will include:



  • Establishing and maintaining relationships with service users, relatives/carers, and colleagues
  • Delivering agreed therapeutic interventions to assist service users in achieving their recovery goals, including improving activities of daily living, social inclusion, and meaningful occupation
  • Undertaking physical health assessments, including weight, height, blood pressure, ECG, and phlebotomy
  • Contributing to the planning and delivery of therapeutic activities with groups of service users as part of the care programme approach
  • Supporting service users in accessing various activities and services to maintain their independence and promote their recovery
  • Ensuring that own actions support the equality, diversity, rights, and responsibilities of individuals and promote independence
  • Reporting and communicating service users' health status and care aims in care records and verbal reports to care coordinators and other members of the multi-disciplinary team
  • Having an awareness of community resources available for service users
  • Involvement in the review of risk and communicating any changes to key workers
  • Collaborating with other Trust services to promote seamless service user-focused care, admission, transfer, and discharge on identified care pathways
