Job Opportunity: Reablement Support Worker

We are seeking a dedicated and compassionate Reablement Support Worker to join our team at Wiltshire Council. As a Reablement Support Worker, you will play a vital role in helping individuals regain their independence and confidence in their own homes.

About the Role

As a Reablement Support Worker, you will work closely with occupational therapists to develop personalized reablement plans for our customers. You will provide vital assistance to individuals in their homes, helping them to achieve their goals and maintain their independence.

Key Responsibilities
  • Provide personalized support to individuals in their homes, helping them to regain their independence and confidence.
  • Work closely with occupational therapists to develop and implement reablement plans.
  • Assist individuals with daily living tasks, such as personal care and household chores.
  • Encourage and support individuals to take control of their own lives and make informed decisions about their care.
  • Work collaboratively with other healthcare professionals to ensure seamless care and support.
