**Job Overview**

We are seeking a highly motivated Mental Health Support Specialist to join our team in West Sussex. This is an exciting opportunity to work with individuals who require targeted mental health and holistic support.

About the Role

This role will involve working closely with clients who have severe mental illness (SMI) to engage with their Physical Health Check outcomes and wider Social Prescribing support needs. As a Mental Health Support Specialist, you will be responsible for supporting clients to improve their health and wellbeing through various means, including linking them into community resources and promoting a holistic approach to health.

Key Responsibilities:

  • Supporting SMI clients to engage in their Physical Health Check outcomes following their annual Physical Health Check within their GP Surgery.
  • Working within the community and various sites in a peripatetic way.
  • Motivating clients to engage with and take ownership of their health and wellbeing.
  • Providing short-term support to a caseload of clients with complex needs.
  • Linking clients into the community using a holistic approach to improve their health and wellbeing.
  • Promoting and supporting the implementation and development of the service.
  • Building and maintaining professional relationships with external partners and stakeholders.
  • Identifying areas of support within the community and mental health services that supports the client's goals.
  • Using in-house and partner's IT systems to record and monitor client information and managing a caseload.

What You'll Need

To succeed in this role, you will need a good understanding of working within Mental Health and Wellbeing, Social Prescribing or equivalent transferable skills. A genuine passion for supporting people to explore their needs is essential. You will also receive a comprehensive induction followed by regular training to ensure you have all the tools needed to succeed.

Salary and Benefits

The estimated salary for this role is £28,000 - £32,000 per annum, depending on experience.
