About the Role
Southern Health NHS Foundation Trust is seeking a highly motivated and compassionate Mental Health Practitioner to join our Crisis and Liaison Service team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises in our acute hospital setting.
Key Responsibilities
- Assess and manage patients with mental health needs in crisis, providing timely and effective interventions to ensure their safety and well-being.
- Work collaboratively with other healthcare professionals to develop and implement individualized care plans, promoting recovery and reducing hospital admissions.
- Provide education and training to healthcare staff on mental health issues, promoting a culture of compassion and understanding.
- Contribute to the development and implementation of service improvement initiatives, ensuring that our service is responsive to the needs of our patients and staff.
Requirements
- Registered Mental Health Nurse, Occupational Therapist, or Social Worker with a strong background in crisis and liaison work.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare staff.
- Knowledge of mental health legislation, policies, and procedures, with a commitment to upholding the rights and dignity of patients.
- Ability to work flexibly, including evenings, weekends, and bank holidays, to meet the needs of our service.
