About the Position

The Patient Care Specialist will work closely with healthcare professionals to provide personalized care and support to individuals in their own homes. This role requires a strong understanding of community care principles and a commitment to delivering exceptional patient outcomes.

Key Responsibilities
  1. Develop and implement personalized care plans in collaboration with healthcare professionals
  2. Provide emotional support and reassurance to individuals and their families
  3. Work collaboratively with the multidisciplinary team to deliver high-quality patient care
Requirements and Qualifications
  • NVQ Level 3 in Health and Social Care
  • Care Certificate
  • Maths & English qualifications
  • 2 years' experience in the Community or equivalent
  • Venepuncture and wound care competencies

Estimated Salary: £24,157 - £30,615 per annum
