About the Role

We are seeking a skilled and compassionate Community Support Specialist to join our Home First Team in Kerrier. As a key member of our team, you will play a vital role in delivering reablement services to support individuals in regaining their independence and confidence.

Key Responsibilities

  • Deliver packages of care to patients in their own homes, including residential homes and nursing homes.
  • Work collaboratively with a range of services to prevent hospital admission and support discharge from hospital.
  • Provide flexible and responsive support to meet the needs of the service, including working outside of regular hours.
  • Develop and maintain effective relationships with patients, families, and healthcare professionals.
  • Contribute to the development of care plans and ensure that patients receive high-quality, person-centered care.

Requirements

  • NVQ Level 3 qualification in care or equivalent experience.
  • Evidence of experience in a care setting.
  • Valid UK driver's license.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
