Responsibilities:

• Monitor patient conditions and update care plans as necessary.
• Collaborate effectively with teams to ensure optimal patient outcomes.
• Provide support and guidance to patients and their families, addressing their concerns and promoting a positive care experience.
• Maintain accurate and up-to-date patient records.
• Adhere to all policies, procedures, and regulatory standards.
