Occupational Therapist

We are seeking a skilled and experienced Occupational Therapist to join our team in adult social care. With a strong background in physical rehabilitation, you will work within an integrated Reablement Service on a rehabilitation programme, supporting service users in the community for up to six weeks.

Your expertise in case work and clinical collaboration will be invaluable in this role, which involves working closely with in-house reablement officers to provide care under your direction. You will manage a varied caseload comprising both hospital discharges and community referrals, and be required to complete risk-assessed home visits as necessary.

Main Responsibilities:

• Work within an integrated Reablement Service on a rehabilitation programme, supporting service users in the community for up to six weeks.
• Manage a varied caseload comprising both hospital discharges and community referrals.
• Complete risk-assessed home visits as necessary.
• Work closely with in-house reablement officers to provide care under your direction.

To be successful in this role, you will need:

• A minimum of two years' experience as a qualified OT.
• Strong case work skills and expertise in physical rehabilitation within a fast-paced environment.
• Excellent clinical collaboration skills.
• Comfort with a flexible work schedule.
