Nurse Care Coordinator

South Coast Nursing Homes is a leading provider of high-quality care services, committed to delivering exceptional resident care and comfort. We are seeking a skilled Registered Nurse to join our clinical team, working closely with our experienced Home Manager to provide proactive and reactive clinical duties.
As a Registered Nurse, you will be responsible for maintaining high standards of patient care, ensuring the health, safety, and welfare of all residents. You will champion patient choice and autonomy, working collaboratively with colleagues to deliver excellent resident care and exemplary communication.
Key responsibilities include:
- Maintaining accurate and comprehensive documentation, including individualized care plans and medication management.
- Ordering and administering medication, adhering to legal, professional, and company policies.
- Supporting the Home Manager in managing the care team, ensuring seamless communication and collaboration.
- Fostering relationships with families, advocates, and NOK, keeping them informed of resident changes.
Essential qualifications include a Nursing degree or above, registration with the NMC, and current active PIN. Desirable skills include prioritization, reporting, and team management experience.
