At CCS Vively Health, LLC, we're seeking a highly skilled Dialysis Nurse Specialist to join our team.

About the Role

We're looking for a motivated and compassionate nurse to provide high-quality patient care in a dynamic and supportive environment. The successful candidate will work closely with our multidisciplinary team to deliver exceptional care and outcomes for patients undergoing dialysis treatment.

Key Responsibilities
  • Provide direct patient care, including assessing, planning, implementing, and evaluating individualized care plans
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between settings
  • Develop and implement educational programs to promote patient self-management and empowerment
Requirements

To be successful in this role, you'll need:

  • A current Registered Nurse (RN) license in the state of practice; minimum 12 months' experience in acute or chronic hemodialysis nursing
  • An Associate's Degree in Nursing (A.D.N) from an accredited school of nursing; Bachelor's Degree in Nursing (B.S.N) preferred
  • Basic computer skills and proficiency in MS Word and Outlook
