Job Overview

We are seeking a highly skilled and experienced Clinical Anticoagulation Specialist Nurse to join our team at Hywel Dda University Health Board.

About the Role

This is an exciting opportunity for a dedicated and passionate nurse to lead the delivery of anticoagulant services in Carmarthenshire, working closely with Consultant Haematologists and other healthcare professionals. The successful candidate will be responsible for managing the delivery of nurse-led anticoagulant services, accepting direct referrals from primary and secondary healthcare professionals, and providing expert knowledge and clinical skills through nurse-led clinics.

Main Responsibilities
  • Manage the delivery of nurse-led anticoagulant services in collaboration with Consultant Haematologists.
  • Accept direct referrals from primary and secondary healthcare professionals for patients needing anticoagulation therapy.
  • Deliver nurse-led care following agreed protocols for the initiation and maintenance of anticoagulation therapy for outpatients with atrial fibrillation.
  • Analyze, coordinate, organize, and monitor the treatment of patients receiving anticoagulation therapy, following agreed protocols, including dose adjustments according to blood test results and managing emergency treatment for patients at risk of thrombosis/bleeding due to under/over anticoagulation.
Requirements
  • Registered nurse - educated to degree level.
  • Live NMC Registration.
  • Teaching Qualification.
  • Counselling skills course or demonstrable experience.
  • Evidence of postgraduate training at master's level in anticoagulation.
  • Independent Nurse Prescriber or evidence that working towards.
Salary & Benefits

The salary for this position is estimated to be around £45,000-£60,000 per annum, depending on experience. In addition to a competitive salary, we offer a range of benefits, including professional development opportunities, flexible working arrangements, and a supportive work environment.
