We are seeking a highly skilled and experienced Team Leader to join our dynamic team at Meridian Health and Social Care. As a Team Leader, you will play a crucial role in ensuring the delivery of high-quality care services to our service users.

Key Responsibilities:
  • Conduct initial assessments for new service users to ensure they receive person-centered care.
  • Report any relevant issues concerning the care of service users to the care manager in a timely manner.
  • Monitor and maintain work standards of a team of care workers, including specialist teams where appropriate.
  • Assist with the maintenance of records, in written or computerized format, as specified within procedures or as requested by care coordinators.
  • Deputize for the care coordinator and care manager in their absence.
  • Provide cover in other areas as and when required.
  • Participate in an on-call rota to deliver care in emergency situations.
