About the Role:

This key position involves working closely with patients, families, and interdisciplinary teams to provide comprehensive support and services.

Key Responsibilities:
  • Develop and implement evidence-based treatment plans, tailored to individual patient needs.
  • Collaborate with healthcare professionals to ensure continuity of care and optimal patient outcomes.
  • Maintain accurate records of patient progress and contribute to quality improvement initiatives.

The ideal candidate will possess excellent communication and interpersonal skills, as well as a strong commitment to equity, diversity, and inclusion.

Requirements:
  • A Master's degree from an accredited school of social work (CSWE).
  • 2-3 years of professional social work experience, including psychiatric emergency, crisis, or outreach work.
  • Licensure as a social worker, as defined by the State Board for Social Work, is required.

The estimated salary range for this position is $63,003 - $80,600 annually, based on market data, education, experience, qualifications, expertise, and internal equity considerations.
