Job Title: Primary Care Physician
Job Summary:
This is a position for newly qualified GPs to work within Hammersmith and Fulham Central PCN on a fixed term contract. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. This role requires strong communication and collaboration skills, as well as the ability to work autonomously and make professional decisions in relation to presenting problems.
Key Responsibilities:
- Manage a caseload and deal with a wide range of health needs in a primary care setting
- Ensure the highest standards of care for all registered and temporary patients
- Travel between different practices within the PCN as required
- Be registered with the General Medical Council (GMC) and hold a certificate of completion of training within two years
- Make professional, autonomous decisions in relation to presenting problems
- Assess the health needs of patients with undifferentiated and undiagnosed problems
- Screen patients for disease risk factors and early signs of illness
- Contribute to QOF requirements and recording data appropriately
- Develop care plans for health-related illnesses
- Screen patients for health education
- Admit and discharge patients to and from the caseload and refer to other care providers as appropriate
- Record clear and contemporaneous consultation notes to agreed standards
- Collect data for audit purposes and contribute to the practice audit programme as required
- Prescribe in accordance with the accepted prescribing formulary
- Take responsibility for some aspects of the clinical work within the team
- Foster and maintain strong links with all services across the PCN and neighbouring networks
- Collaborate with other healthcare professionals to provide high-quality patient care
- Work with your line manager to undertake continual personal and professional development
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety
- Review yearly progress and develop clear plans to achieve results within priorities set by others
- Promote diversity and equality in people management techniques and lead by example
Requirements:
- Recognises priorities when problem-solving and identifies deviations from normal pattern and can refer to seniors when appropriate
- Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
- Understands and demonstrates the characteristics of a role model to members in the team and/or service
- Demonstrates self-development through continuous professional development activity
- Demonstrates ability to improve quality within limitations of service
- Demonstrates ability to motivate self to achieve goals
- Promotes diversity and equality in people management techniques and leads by example
