We are seeking an experienced clinical pharmacist to join our multidisciplinary team at Holland House Surgery, a dynamic and forward-thinking practice serving a population of around 11,000 patients in Lytham.

The ideal candidate will have excellent communication skills, a strong analytical mind, and a passion for medicines optimisation. They will be responsible for conducting face-to-face and telephone medication reviews, answering medication-related queries, and optimising medicines to manage chronic diseases.

This is a challenging and varied role that offers opportunities for professional development and growth. Our team provides a supportive environment where you can work collaboratively with GPs, Advanced Practitioners, Nurses, and other healthcare professionals to deliver high-quality patient care.

Key Responsibilities

  • Conducting medication reviews with patients and providing recommendations for improved medication management
  • Answering medication-related queries from patients and other clinicians
  • Optimising medicines to manage chronic diseases, including hypertension, diabetes, and respiratory conditions
  • Managing acute medication requests and ensuring timely dispensing
  • Liaising with our established PCN pharmacy team to ensure seamless integration of services
  • Participating in virtual and in-person group consultations to promote best practices in medicines use

Requirements

  • Qualified Pharmacist registered with the General Pharmaceutical Council
  • Independent Prescriber or working towards this qualification
  • Experience of working in general practice or general practice IT systems (desirable)
